In accordance with Texas Health & Safety Code Section 181.105 and House Bill 4224, the following information is provided regarding your rights to access your health care records, contact licensing authorities, and file consumer complaints.
1. Requesting Your Mental Health Care Records
You have the right to request a copy of your mental health records maintained by the NTCC Counseling and Wellness Center. To request your records, please follow these steps:
- Submit a Written Request: Deliver a written request in person directly to the Director of Mental Health Services in the Counseling and Wellness Center or via email to counseling-center@ntcc.edu.
- Required Information: Your request must include your full name, student ID number, date of birth, contact information, and a specific reason and description of the records you are requesting.
- Processing: Requests will be processed in accordance with state and federal law (FERPA and HIPAA, as applicable at Northeast Texas Community College Counseling and Wellness Center).
2. Contacting the Licensing Authority
The Licensed therapist(s) at NTCC are regulated by the Texas Behavioral Health Executive Council (BHEC). You may contact BHEC to verify a license or ask questions about professional standards:

3. Filing a Consumer Complaint

4. Record Management Policy
Under Texas Health & Safety Code Section 181.104, our center maintains records securely for a minimum of five years following the last date of service. While records are confidential, they may be disclosed as required by law (e.g., in cases of imminent danger or court order).
